The Physical Therapist will play a crucial role in providing
high-quality rehabilitation services to our residents, with a focus
on restoring mobility, improving strength and function, and
enhancing overall quality of life.
Weekdays/Weekends business hours
Primary Responsibilities and Duties:
Evaluate Residents' physical abilities, functional status, and
rehabilitation needs through comprehensive assessments.
Develop personalized treatment plans based on assessment findings,
resident goals, and evidence-based practice guidelines.
Implement therapeutic interventions to address impairments,
maximize functional independence, and promote safe mobility.
Provide one-on-one physical therapy sessions to residents,
utilizing a variety of therapeutic techniques and modalities.
Monitor resident progress and adjust treatment plans as needed to
optimize outcomes and achieve rehabilitation goals.
Collaborate with team members, including physicians, nurses,
occupational therapists, and speech-language pathologist, to
coordinate care and ensure continuity of services.
Educate residents and their families on exercise programs, safety
techniques, and home exercise regimens to support ongoing
rehabilitation and prevent re-injury.
Document all physical therapy services provided, including initial
evaluations, progress notes, and discharge summaries, in accordance
with facility policies and regulatory requirements.
Participate in resident care conferences, team meetings, and
in-service training sessions to share expertise, contribute to care
planning, and stay informed about best practices in
rehabilitation.
Contribute to a positive and supportive work environment by
demonstrating professionalism, teamwork, and effective
communication skills.
Perform all other duties assigned.
Job Qualifications:
Current state licensure or eligibility for licensure as a Physical
Therapist.
1 year Physical Therapist experience
Clinical experience in a skilled nursing facility or similar
healthcare setting is preferred.
Strong clinical reasoning and decision-making skills, with
proficiency in evaluating and treating a diverse range of
conditions and diagnoses.
Ability to build rapport with residents through a compassionate and
caring demeanor.
Excellent interpersonal and communication skills, with the ability
to collaborate effectively with residents, families, and team
members.
Commitment to providing high-quality, resident-centered care with
empathy and compassion.
Requirements:
Physical Requirements:
Work performed in a medical setting due to the need to work
directly with residents.
Ability to stand, walk, and move around the facility for extended
periods.
Frequent bending, stooping, reaching, and lifting of therapy
equipment and assisting residents.
Manual dexterity and coordination for performing therapeutic
exercises, manipulating therapy tools, and conducting
assessments.
Strength and endurance to assist residents with mobility,
transfers, and positioning as needed.
Visual acuity and auditory perception to observe and assess
resident movements and communicate effectively with residents and
team members.
Stamina and physical resilience to handle the demands of providing
therapy services to multiple residents throughout the day.
Compliance with infection control protocols and safety precautions
to minimize the risk of injury or illness to oneself and
others.
Flexibility to adapt to various environmental conditions and
accommodate the physical needs of residents with diverse mobility
levels and functionality abilities.
Ability to operate therapy equipment and assistive devices safely
and effectively, following proper training and protocols.
Maintain regular attendance and timeliness as set forth within the
work schedule designated by the department supervisor/manager.
The job requires accepting criticism and dealing calmly and
effectively with high-stress situations.
The ability to use your abdominal and lower back muscles to support
part of the body repeatedly or continuously over time without
"giving out" or fatiguing.
