Job Description:
A company is looking for a Care Coordinator, Remote in MI.
Key Responsibilities:
Serve as primary care manager for members with high medical
needs
Engage members to complete comprehensive needs assessments and
develop individualized care plans
Coordinate with internal teams and community resources to support
member care and monitor progress
Required Qualifications:
Active, unrestricted RN licensure in the state of Michigan
2+ years of experience with diverse populations and complex medical
needs
1+ years of community outreach or field work experience in a
healthcare setting
1 year of medical case management experience
Access to a designated quiet workspace in the home and must reside
in Michigan
